This is a program that reimburses you, as a caregiver, of persons with dementia for respite services. The program is designed to give you a break from caregiving, so you have some time to care for yourself.
You will be responsible for the selection, arrangement, and payment of the respite services through a licensed in-home provider, a licensed assisted living facility, or an adult day care facility.
The Community Care Services Program (CCSP) is our flagship program at Direct Services and assists seniors and individuals with physical disabilities to live in their own homes or with their families or caregivers.
The Home Delivered Meals Program provides meals, nutritional screening, assessment, education, and counseling to you, as a caregiver, and your family member receiving this service.
The target group for these services are persons who are functionally impaired in their ability to perform regular activities of daily living and activities related to maintaining the home.
