COMMUNITY CARE SERVICES PROGRAM

Who is eligible for CCSP?

The Community Care Services Program (CCSP) assists seniors and individuals with physical disabilities to live in their own homes or with their families or caregivers. It offers a less costly option to nursing home placement because your loved one can be cared for at home.

Community Care Services Program clients must meet the same medical, functional, and financial criteria as clients receiving nursing home care under Medicaid.  Direct Services’ goal with this program is to provide resources to support the client remaining in their community as long as possible.

What Services Are Available Under CCSP?

Care Coordination

A registered nurse and a social worker assess your loved one’s medical, functional, and social needs. They will connect you to service providers and resources while monitoring the services as they are provided.

Adult Day Health

This program is designed to enrich your loved one’s health and social well-being. Adult Day Health provides a variety of health, therapeutic, and social service activities in a group setting. Activities may vary by location.

Consumer-Directed Personal Support

This program provides access to the same services in the Personal Support Services program, but the client or caregiver will hire and supervise the worker(s) of their choice.

Alternative Living Services

24-hour health support is key in this program. Your family member will be provided with 24-hour supervision, medically oriented personal care, routine nursing supervision, and health-related support services in a residential setting other than the client’s home. This service is provided in state-licensed personal care homes.

Structured Family Care

This service is designed to support live-in family caregivers unable to work outside of the home with health coaches and provisions of a daily stipend.

Emergency Response System (ERS):

Your family member’s residence will be equipped with in-home electronic support systems for two-way communication 24 hours a day.

Home-Delivered Meals

This program ensures that your family member receives adequate nutrition to enhance their health. All meals are prepared under the nutrition standards selected by the Georgia State Board of Health. Meals are delivered Monday through Friday, and you are able to receive both hot meals and shelf-stable meals according to your preference. Are meals equipped for certain dietary needs/restrictions?

Home-Delivered Services

Your family member will receive necessary healthcare in their home without having to travel to meet with professionals. These home-delivered services include:
  • skilled nursing
  • physical therapy
  • speech therapy
  • occupational therapy

Personal Support Services

Your loved one will have access to a range of support services through this program including:
  • housekeeping
  • errand running
  • feeding
  • dressing
  • bathing
  • movement assistance around the home

Not at this time. We must first determine if you are eligible to receive these services through Medicaid before services can begin.
  • Contact a River Valley AAA Gateway Specialist at 706-256-2900 or 1-800-615-4379 for a phone assessment to determine your eligibility for service.
  • When a space becomes available you will be referred to Direct Services to schedule an initial assessment at your home.
  • Service will begin within 5 business days of the initial home visit.
